1. **Acquire the application form:** Either download and print the prescribed form from the official website, or collect it in person from the District Industries Centre.

2. **Fill in the application:** Enter accurate details in every mandatory field. Affix a passport-sized photograph, signing across it if so required. Enclose self-attested copies of the documents called for, such as proof of business registration, product particulars, and address proof.
